What is it?
The best way to maintain your independence and live life on terms that suit you is by having a support service in your own home. The Outreach support service is designed for people who need additional support with day-to-day household tasks, personal care or any other activity which allows them keep living in the quality manner they want while still being able be most self-reliant possible.
We develop a team and support plan and offer as little or as much support you need within the comfort of your own home. This could include supporting you access the community, maintaining your home or support with medication.
How it works
- Our support Outreach support service is carried out through a robust and thorough assessment of your needs where you have direct input.
- Once a care and support plan has been agreed we will provide you with qualified and trained staff members who will follow the support plan you help set out!
- The length and frequency of the support is flexible we are happy to drop in for 1 hour a week or for those who require more support we can provide 24-hour care and everything in between!
